About Your Protected Health Information

Hiawatha Valley Mental Health Center respects each client's right to the confidentiality and privacy of their healthcare information.  We will make every attempt to protect the privacy of this information so that it not shared with others for any other reason than to promote the best outcomes for our clients.

The agency requires a signed authorization from the client or legal guardian / representative to disclose any protected health information.  Client records are the property of Hiawatha Valley Mental Health Center and they are maintained for the use of the agency and its staff and for the benefit of our clients.  You may request a copy of your records at anytime, with reasonable notice and payment of copying costs.

General Billing Information

Hiawatha Valley Mental Health Center accepts most major insurance companies.  As a courtesy to our clients, Hiawatha Valley Mental Health Center will bill your insurance company directly, provided that we receive all of the necessary information to do so.

Our staff are credentialed with the following insurance companies:

• Blue Cross Blue Shield of Minnesota

• Health Partners

• Mayo Management (MMSI)

• Medicare

• Minnesota Health Care Programs (Blue Plus, UCare, Minnesota Medical Assistance)

• Preferred One (Cigna Behavioral Health & Wausau)

• United Behavioral Health

• Value Options

• Veterans Administration

In addition to accepting most other major insurances, we also offer services on a sliding fee scale to those individuals who qualify.  Under this option, payment requirements will be based on verified gross income, number of dependents, and county eligibility.
