Community Based Services

Services to clients with serious and persistent mental illnesses include (SPMI):

Community Supportive Care Workers: Provides individuals an opportunity to develop social and friendship skills.

Community Support Program: CSP assists with primary aspects of daily living such as housing, personal relations, medical care, income, work skills, recreation and transportation.

Targeted Case Management: Specializes in coordination of services, ongoing monitoring and evaluation for serious and persistent mentally ill adults.

 

Service to clients with a serious mental illness are:

 Adult Rehabilitation Mental Health Services: The Adult Rehabilitative Mental Health Services (ARMHS) program was a new addition in 2002. Services can be provided on an individual basis or by groups. Also referred to as the Rehab Option, this program is a Medical Assistance billable service similar to the Community Support Program. Services consist of skill building training in independent living skills, social skills, and medication education.

Community Alternatives for Disabled Individuals (CADI) Case Management: CADI Case Manager works with Community Health Services or County Human Services Case Managers to coordinate services, do ongoing monitoring and evaluation of services for individuals receiving CADI waiver funding.

Day Treatment: Consists of a structured program of group psychotherapy and teaches independent living skills. These services are provided by a multi-disciplinary staff, two days per week in Winona County.  There are no Day Treatment Services in Houston and Wabasha Counties.

Medication Team: This is a group of CBS staff that work with the State Initiative Registered Nurse and an LPN to assist individuals in receiving their medications on a daily or weekly basis depending on the individual’s needs.

Pre-Petition Team: This team works in Winona County to handle the screenings for cases that require commitment proceedings. The team works closely with the County Attorney’s Office in ensuring the safety of the individual and the community.

Vulnerable Adult Targeted Case Management: Case Managers specialize in coordinating services, ongoing monitoring and evaluation for the Vulnerable Adult.
