Adult Community Based Services

Targeted Case Management:

Specializes in coordination of services, ongoing monitoring and evaluation for serious and persistent mentally ill adults.

Community Support Program (CSP):

This service is a grant-based program funded by the County of Financial Responsibility (COFR) for serious and persistent mentally ill individuals. CSP offers assistance and support with benefits, psychosocial rehabilitation, medication monitoring, transportation, activities of daily living, mental health support, and housing support. Admission to this program requires prior approval from the COFR to obtain funding.

Adult Rehabilitation Mental Health Services:

The Adult Rehabilitative Mental Health Services (ARMHS) program was a new addition in 2002. Services can be provided on an individual basis or by groups. Also referred to as the Rehab Option, this program is a Medical Assistance billable service similar to the Community Support Program. Services consist of skill building training in independent living skills, social skills, and medication education.

Hiawatha Valley Mental Health Center is currently certified by DHS to provide ARMHS Services in Wabasha, Winona and Houston County.

Mobile Crisis Services:

Mobile Crisis is a Medical Assistance funded program currently available in Houston, Wabasha, and Winona Counties seven days a week, 24 hours a day. Mental health professionals and practitioners are available to assist with crisis calls. One crisis bed is available within the three counties. Mobile Crisis Services can be obtained by calling Hiawatha Valley Mental Health Center on Monday-Friday, 8:00 AM-5:00 PM at 507-454-4341 and Evenings, Weekends, and Holidays, at 1-800-657-6777.
