Hiawatha Valley Mental Health Center provides Children’s Mental Health Case Management to youth living in Winona and Fillmore Counties. The services coordinate with the client and family to assist with accessing community support services. The services provide activities designed to help each child with a severe or emotional disturbance be able to function and remain with his/her family in the community. Case managers assist families in navigating the mental health field, assisting in referrals to additional services and resources.
In order to be eligible for Children’s Mental Health Case Management, a youth has to meet at least one of the following criteria:
- The youth has been admitted within the last three years or is at risk of being admitted to inpatient treatment or residential treatment for an emotional disturbance
- The youth is a Minnesota resident and is receiving inpatient treatment or residential treatment for an emotional disturbance through the interstate compact
- The youth is diagnosed by a mental health professional as having clinical depression or psychosis, is at risk of harming self or others, or has psycho-pathological symptoms as a result of being a victim of physical or sexual abuse or of psychic trauma during the past year
- The youth, as a result of an emotional disturbance, has significantly impaired home, school, or community functioning that has lasted one year, or in the written opinion of a mental health professional, presents substantial risk of lasting at least one year
