The following measures are in place and are required prior to being seen in person for services either in the community or in our agency settings.

  • All consumers prior to being seen for their initial face to face visit will be asked to review and sign the Assumption of Risk and Waiver of Liability Related to COVID-19 form
    • If a consumer refuses to sign the form, they will not be seen in-person
  • Prior to each visit, consumers will be asked the following:
    • Are you having any COVID related symptoms?
    • Do you have a temperature of 100.3 or above?
      • If you answer “yes” to any of the following questions, you can not be seen for in person service
  • Face coverings are required at all of our clinic locations
  • Face coverings are required when traveling with a Hiawatha Valley Mental Health Center staff member
  • Whenever possible, everyone should leave at least six feet (about two meters) to the person closest to them
  • Frequent hand washing with soap is vital to help combat the spread of any virus
