School Linked Mental Health Skills and Children’s Mental Health Case Management position

The dual position will be an equivalent to Full-Time.  The services will be provided at home, school, community, or natural setting for the child/adolescent.

School Linked Mental Health Skills: The SLMH Skills piece provides school based mental health assessment, therapy, skills and training, consultation, referral services and support services for children, adolescents and families.  The incumbent will complete diagnostic assessments, facilitate student groups on relevant issues when appropriate,  complete documentation, billing and maintain case files, participate in client IEP meetings and other client related school meetings with parent(s)/guardian(s) and other providers.  Develop and maintain strong working relationships with school personnel and county agency staff. The CMH Case Management pieces provides direct therapeutic skills to children/adolescents with functional skill deficits due to mental health needs.    Incumbent will attend all clinical supervision and client consultation meetings as required.  The direct client care is based on SLMH and/or CTSS guidelines.  Develop and maintain treatment plans that address the skills needed for individual clients based off of therapist’s diagnostic assessment and recommendations.

Children’s Mental Health Case Management: Children’s Mental Health targeted case management (CMH-TCM) services help children with severe emotional disturbance (SED) gain access to medical, social, educational, vocational and other necessary services connected to the person’s mental health needs. Targeted case management (TCM) services include developing and completing assessments and support plans based on the child’s individual needs, referring and linking the child/family to mental health and other services, ensuring coordination of services, and monitoring the delivery of services.

Incumbent must hold a valid driver’s license and maintain valid auto insurance.  Must possess a minimum of a bachelor’s degree in social work or related field, hold a mental health licensure or licensure eligible.
