Youth Targeted Case Management

Our Target Case Managers work with the youth and family to develop a plan that best fits the youth’s mental wellbeing.  Through the team approach, we coordinated activities designed to help the youth with severe emotional disturbance and the youth’s family obtain needed mental health services, social services, health services, vocational services, recreational services and related services in the areas of volunteering, advocacy, transportation and legal.
